Reablement Care


Reablement care is provided by specially trained care staff who work with clients after an unplanned hospital stay or illness. Rather than undertaking tasks for you, care and support staff will work with the client to enable them to increase their level of independence at home and in the community. This may mean supporting a client to re-learn skills that might have been lost while unwell. Very often the service will run alongside a traditional homecare service. The service is usually free for 6 weeks. For more information, please speak with your Social Worker or hospital discharge team.
